Southeast provides evidence-based healthcare services and supports people in becoming active participants in their personal healthcare decisions. Our holistic approach addresses physical, behavioral healthcare and life and wellness needs together, in order to help our patients live longer, healthier and more fulfulling lives.

Southeast staff are here to help with your healthcare needs. We have convenient hours and provide services at several locations. If you have insurance, Medicaid or Medicare, we will bill your insurance and you pay your copay. If you have no insurance, the cost of your care will be based on your income and where you fall on our sliding fee scale.

Primary Care Services in Franklin, Belmont, Harrison, and Monroe Counties

Southeast coordinates and integrates your care across services and healthcare locations at our sites and at community health and social services locations. We also offer Healthcare Services for people who are homeless, living in transitional housing, or who have been previously homeless. If you need services we don’t provide, we’ll help assist with referrals and coordination of care. Walk-ins are welcome within our capacity.
